I don't understand this sentence from a book? help plz?

It's called "under the belt" when someone attacks a guy in the crotch (because it's kind of a weak point). So for a girl getting hit in the breast could also be considered a sensitive/weakspot and thus also "under the belt" even though it's not literally lower than the belt anymore.
